Developing a clinical screening tool for frailty in the acute care setting.

چکیده
A frailty assessment tool was designed, based on evidence from current studies. The tool has fi ve criteria. These include age (75 and over), fall with any injury or fracture (excluding fracture of the neck of the femur), dementia or delirium, care home residence, and reduced or lack of mobility over 24 hours. If any acute elderly inpatient meets at least three of these fi ve criteria, this patient should be diagnosed as having frailty, indicating a need for a comprehensive geriatric assessment. The background rationale for establishing these criteria is as follows.

Finding consensus on frailty assessment in acute care through Delphi method
OBJECTIVE We seek to address gaps in knowledge and agreement around optimal frailty assessment in the acute medical care setting. Frailty is a common term describing older persons who are at increased risk of developing multimorbidity, disability, institutionalisation and death. Consensus has not been reached on the practical implementation of this concept to assess clinically and manage older ...
متن کاملFrailty Screening Tools: Frail Detection to Primary Assessment
Frailty is a vulnerable situation among older adults which can lead to unfavorable health outcomes such as falls, mortality, functional decline and institutionalization. The increasing number of elderly people and low rate of mortality has necessitated the need for high-quality medical services for this aging population, and this has led to a high cost of geriatric health care. There exist a hu...
متن کاملEvaluating the Implementation and Feasibility of a Web-Based Tool to Support Timely Identification and Care for the Frail Population in Primary Healthcare Settings
Background Understanding and addressing the needs of frail persons is an emerging health priority for Nova Scotia and internationally. Primary healthcare (PHC) providers regularly encounter frail persons in their daily clinical work. However, routine identification and measurement of frailty is not standard practice and, in general, there is a lack of awareness about how to identify and respond...
متن کاملThe clinical frailty scale predicts functional decline and mortality when used by junior medical staff: a prospective cohort study
BACKGROUND Increasing frailty is associated with risk of mortality and functional decline in hospitalized older adults, but there is no consensus on the best screening method for use by non-geriatricians. The objective of this study is to determine whether the clinical frailty scale (CFS) can be used to identify patient baseline frailty status in the acute general medical setting when used by j...
متن کاملUnderstanding Clinical Importance of Frailty and Designing a Frailty Assessment Tool in Acute Hospitals
Introduction: Managing the acute admission of older patients is a challenge in hospitals. Frail older people represent a larger proportion of acute admissions. Length of inpatient stay, inpatient mortality and the 90-day readmission rate are significant in this group of patients. In order to reduce the risks of adverse outcomes, frailty should be screened using an appropriate assessment tool an...
